"In consideration The Petroff Companies, Inc., is hired to perform a service of which is hereby acknowledged, the undersigned hereby releases The Petroff Companies, Inc. and its officers, directors, employees, agents, subsidiaries and shareholders from any and all claims, demands and causes of action, known and unknown, in law or in equity, for personal injury, property damage or economic loss of any description which the undersigned has or may have on account of anything done or occurring prior to the date hereof service.  The undersigned acknowledges that it and its affiliates have hired to perform a service described in the unsigned submission.

The payments of all The Petroff Companies, Inc. invoices are due at terms of invoices sent.  Interest at a rate of 1.5% per month will be added to any balance unpaid more than 30 days after the date of the invoices.  If a balance remains unpaid for more than 60 days, The Petroff Companies, Inc. will take further action as permitted by law.

The Petroff Companies, Inc. may recover all attorney fees incurred in relation to or in connection with the unpaid balances.  All returned checks are subject to a $25 processing fee.  In the event a dispute should arise between parties to this contract, the parties agree to submit this matter to binding arbitration.  The parties agree to share equally in the costs of the arbitration. All payments are due at terms of invoice. This does not constitute an admission of liability or of any fact by any person or entity released hereby."
